In order to get working papers, please do the following:

• Complete the  top (Part I only) of this form  
•    signed and stamped by the doctor's office
• A copy of your Birth Certificate, Passport or any  Government Photo ID document

Once ALL of the forms above have been completed, then you can bring  it to Ms. Ortiz into Main Office, Room 101.

Please note that it takes 24 hours to process.
